The Intake Process

Requesting an Appointment

If you would like to request a new client appointment, please see below to view pricing by completing the Pre-Feel Form. All clients interested in scheduling must complete the Pre-Feel Form to acknowledge that you have reviewed our costs for services. Planted Hope is a self-pay only, private practice and we do not accept any insurance plans at this time.

Feeler Form

The Feeler Form is a brief questionnaire that provides Planted Hope with general information and treatment needs for you (or the person being referred for services) to ensure Planted Hope is best suited to meet your clinical needs. Once submitted, Planted Hope will reach out via email to advise next steps.

Client Portal

Once Planted Hope reviews the Feeler Form to confirm we are a good match, we will provide an email within 48 hours to complete your intake paperwork and assessment measures via your confidential, client portal, courtesy the Simple Practice platform. All items must be reviewed, signed, and completed at least 72 hours prior to the scheduled appointment or the appointment will be canceled/rescheduled.

Requesting an Appointment for Minors

A parent/guardian must attend the first session for all Planted Hope clients under the age of 18 years old. If a parent/guardian is unavailable for the first session, the session will be cancelled/rescheduled until a parent/guardian is available. 

Payment Methods 

We accept all major credit cards and debit cards. We also accept FSA and HSA cards as well. All payments are due at the beginning of each session. 

Out-of-Network Insurances:

If you are using out of network (meaning your insurance plan is not accepted by the agency or provider), you can pay the full service rate and our office can electronically issue a monthly superbill (statement for reimbursement) via your confidential, client portal. Then, you will submit this to your insurance company for reimbursement. Planted Hope is not responsible for negotiating a settlement with your insurance on a disputed claim or unpaid superbill. 

Reduced Rate:

Upon request, 10% off full cost of service excluding initial evaluations, assessments, family psychotherapy, group psychotherapy, missed appointments, and services provided outside of sessions. Your provider reserves the right to limit the number of reduced rate clients accepted. You do not need to show verification but an agreement will need to be signed. The reduced rate percentage may change without notice to the public, but will be updated on this site. Active clients will be informed with at-least a 30 days notice.​​
